IN AN EFFORT TO PROVIDE A MORE HEALTHY, SAFE AND SATISFYING ENVIRONMENT TO OUR EMPLOYEES, INDIVIDUALS TO WHOM JOB OFFERS ARE EXTENDED MAY UNDERGO NICOTINE TESTING AS A PART OF THE PRE-EMPLOYMENT PROCESS. Coordinates compliance with Core Measure Requirements at hospital level in conjunction with facility core measure coordinator, quality manager and hospital administration. Actively participates in concurrent case review to assure that evidence based best practices are implemented timely for core measures patients. Tracks compliance of measures and provides staff education relating to the externally reported measurements. Assists in the planning and implementation of interventions to ensure evidence-based practices are implemented (Core Measures for AMI, HF, Pneumonia, SCIP). Works closely with providers, nurses and other members of the organization by demonstrating effective communications, teaching and negotiating skills. Is accountable for quality assessment and efficient managed care. Completes established competencies for the position within designated introductory period. Other related duties as assigned. Qualifications MINIMUM EDUCATION: Graduate of an accredited school of nursing for licensed vocational nursing. MINIMUM EXPERIENCE: 3 years combined experience in hospital clinical practice, management and/or clinical education in a healthcare setting with demonstrated expertise in caring for patients within a designated practice area. Working knowledge of hospital regulatory standards, Core Measures and National Patient Safety Goals. Working knowledge of Microsoft Office. PREFERRED EXPERIENCE: 5 years experience in a designated practice area REQUIRED CERTIFICATIONS/LICENSURE: Possession of current Texas State License for Vocational Nurse. REQUIRED COURSES/ COMPLETIONS (e.g., CPR): BLS-Obtained through approved American Heart Association Training Center or the Military Training Network
